Why CPHC?

Who are we? And what might our care look like for you as a patient?

We are a team of people composed of a physician, two medical receptionists, two medical assistants, and two nurses. We answer our own phones when the office is open. If you call and get a voicemail--either all of our lines are busy or we are at lunch. Thank you for your patience if this is the case. Please call back if you do not care to leave a voicemail.

Rosie is one of our receptionists. She makes doing three things at once look easy because she has such a ready smile--you can hear it in her voice on the phone.

Maria is a calm steady presence on our team and she is Rosie’s partner at the front desk.  She is a persistent problem solver, who will make sure our patients have answers to a myriad of practical questions.

Patti, is an experienced medical assistant and a professional clown.  On any given day you might see a small child leave the office beaming, carrying one of her famous balloons.

Missy, one of our nurses, is one of most encouraging and happy people you will ever meet.  She works hard to make sure that things run smoothly for our patients.

Dr. Crevier is a quiet mannered physician.  He deeply enjoys getting to know his patients and is genuinely interested in them.

Carol, who is a nurse,  is back in school, so she mostly is in the shadows of CPHC at the moment.

 

Our staff will be caring deeply for you and about you if you are our patient because we hold that you are a precious human being.

 

We are abundantly blessed to be in private practice making independent medical decisions with our patients based upon what is best for them.  Everyday that we have the privilege to care for interesting and beautiful human beings is a great day at The Center for Primary Healthcare.

No bureaucracy here.  Just a team that works hard for their patients every day.

 

We are affiliated with the University of Chicago Hospitals, particularly Ingalls Hospital.

We personally attend to patients in local nursing homes and rehabilitation centers.
